Bladder and bowel referral form

Unfortunately, we are currently experiencing issues with our website referral form. To maintain patient safety, we will only be accepting referrals via GP surgeries until this issue has been resolved. We hope to rectify the problem as soon as possible and apologise for any inconvenience this may cause.

GPs please use the old referral form via DXS  ‘Referral to continence care (MED1044)’.
