What we do

The community nursing teams provide nursing care for patients, aged 18 and over in their home within the Medway area, delivering personalised care to those with complex and long term conditions, supporting people to stay safe and well at home. The service may provide:

  • wound management
  • change wound dressings
  • palliative, end of life care and symptom control
  • catheter care and provide health advise
  • They also take blood, administer injectable and intravenous medication.

Working hours

The service operates 24 hours a day, 365 days in the year.

Who are we?

We are healthcare professionals, skilled in caring for people in their own homes.

These include:

  • clinical leads
  • community and neighbourhood nurses
  • nurse associates
  • health care assistants
  • phlebotomists (who take blood).
