We can use the F number or a form for blood tests

If you are coming for a blood test please bring a blood form with you. We are currently unable to use the F number due to an issue with the DART system. This is being investigated.

Please get the form from your GP or clinician. You will need to rebook your appointment if you are unable to get one.
