A number of our services can also receive referrals via a dedicated referral line, ‘MCH OneCall’.

MCH OneCall means that GPs, practice nurses, social care and hospital teams can now just call one number to refer patients who need support from our community nursing, rehabilitation, therapy, respiratory, diabetes, cardiology, phlebotomy or medication services.

The MCH OneCall team can be contacted on 01634 891900 and will need the following information to process a referral:

•    Patient demographics (including GP surgery)
•    A contact name and number for the referrer
•    The reason for the referral
•    Any other relevant medical/social history
•    Any special notes (including access to patient’s home if restricted, next of kin if needed etc)
    
Using MCH OneCall means that you don’t need to fill out a referral form. Details of the patient will be recorded and transferred to the appropriate service for clinical triage – helping to simplify referrals and make sure our patients receive the right care at the right time. 

MCH OneCall gives you an additional option for referrals and does not replace existing contact arrangements you may already have in place should you prefer to use them.
