What is intermediate care?

We aim to give you the support you need when you are being discharged from hospital. Each individual is assessed and where possible supported to return home. For some people who cannot return home immediately from hospital, we will continue your rehabilitation in one of our dedicated service locations such as Amherst Court for example.

Home FirstWhat happens if I go home?

If you go home from hospital and need some additional support on discharge you will be assessed under our Home First programme to ensure you have the right level of support to keep you safe and well. This may include:

  • specialist equipment
  • an individual rehabilitation programme
  • assistance with personal care

Ambley Care logoOur team of personal carers and enablers are provided by Ambley Care, our partner organisation.

They work under the guidance of the rehab therapy team, taking responsibility for the physical and emotional wellbeing and social needs of our patients for up to a maximum of six weeks. If within this time it seems like you will need longer term care we will start the ball rolling with a referral to social care for a financial assessment.

What if I can’t go home straight away?

If you need more support and rehabilitation before returning home from hospital you will stay at one of the dedicated units in Medway such as Amherst Court. Our aim is to continue to help you get back home as quickly as possible.

At the end of your stay with us you, your family and carers will help you to arrange any further care or additional support required, if necessary. Our ultimate aim is to get you back to your maximum level of independence as much as possible.
