Heart failure service

The heart failure team helps people with a type of heart failure known as left ventricular systolic dysfunction (LVSD) to have the best quality of life.  We aim to help people understand their condition, optimise medication, advise on beneficial lifestyle changes and support individuals at the end of their life in conjunction with the hospice and community teams.

Once a diagnosis of LVSD heart failure has been confirmed the heart failure nurses will discuss a wide range of topics with the patient, and assess their individual needs.  We can refer individuals for extra support at home if required or to the cardiac rehabilitation team for an individualised exercise programme.

Clinics are held at various locations throughout Medway but home visits are also available to patients who are unable to attend clinic venues.

Referral can be from GPs or hospital consultants by letter or referral form. GPs should arrange for people to have blood tests and chest x-rays before referring to the service.
