The family and carers support team are based at the Wisdom Hospice the team consists of three social workers and one specialist counselor.
Our role is to provide advice, listening support/counselling to patients/families/main carers that are known to the multi-professional team here at the hospice. We also work with children within schools as and when required.
Also as part of our wider team we have a well-established bereavement service and a welfare benefits adviser who offers advice and support with claiming entitlement.
We are able to visit patients and families at home and can support patients and families during a stay at the hospice.
